Every process contains inherent risk. Process safety is no different.

This presentation will present risks inherent to the execution of OSHA's Process Safety Management as identified by a HAZOP of PSM procedures written to comply with the requirements of 1910.119 - Process safety management of highly hazardous chemicals.

Further, the presentation will present the network of information communications within Process Safety and identify likely failures of channels of communication within the context of Process Safety Knowledge Management (PSKM). Specifically with regard to the following steps:

  • Capture Knowledge
  • Organize Knowledge
  • Maintain Knowledge
  • Provide Knowledge

Finally, the presentation will conclude with a brief introduction to the application of Pattern Language principles and methods to Process Safety, with the goal of identifying recurring problems and their solutions at varying scales within the process safety management system.
