(32k) Review of Changes in API Standards and Learnings from Incidents

Learnings from the past industry incidents or near misses can be used to strengthen the existing Process Safety Management (PSM) systems thereby reducing likelihood of hazardous scenarios and potential relief event. Root cause analysis shows gaps in the existing Process Safey Management system and poor implementation. Standards have incorporated these learnings requiring harder look at certain overpressure scenarios during evaluation while designing relief system as a mitigative measure. This presentation delves deeper into these incidents, learnings, and changes to standards to further strengthen PSM system.
