As process safety continues to mature across organizations that have gone through multiple cycles of PHA revalidations and PSM audits, the industry continues to experience incidents and loss events. As process safety professionals, what is being missed? This presentation will highlight a series of common gaps that continue to challenge process safety professionals. These are sometimes missed opportunities to dig into details of safety systems, erroneous assumptions about program strengths and weaknesses, assumptions which are made about systems that are present, or tiny traps that are hiding just under the surface of process safety programs. A list of top 10 hits for common misses in overall process safety systems will be reviewed, and what to ask to be aware of and hopefully close such gaps. The presentation will highlight these gaps with incidents that are related to each concept in a mini case-study manner.
