Why We Keep Having Incidents

Abstract

After every major process safety incident, one of the questions that invariably surfaces is “why do we keep having incidents?” In this paper and presentation, nine “I” factors will be presented as potential answers to this question. These factors, (which include inexperience, intentionality, and impermanence, amongst others) will also be analyzed for potential answers to the related question, “What can we do to break the incident cycle?”
