This session seeks papers illustrating practical ways to optimize incident investigations, and to better apply hard-earned knowledge gained from experience. Potential topics include: 1. Novel applications or unique developments in Root Cause Analysis 2. Getting the most from Process Safety Near Misses 3. Seeking and correcting management system shortcomings before tragedy strikes 4. Mining databases and archives for hints on inherent safety and best practices 5. Do's and Don'ts in writing incident investigation reports 6. Expanding lessons learned across an organization 7. Passing process safety knowledge from Baby Boomers to Millennials
