(10a) “Well the Team Has Met and Completed the PHA….Now What?”

Learning from the experiences and expertise of others has long been recognized as a valued process when completing PHAs.  The gathering of a multi-disciplinary review allows facilities to succeed in a path of continuous improvement by evaluating the current and future state of the process based on the implementation of documented advancements of the safety system.  This is one plant’s journey to success through the realization of gaps in this documentation discipline.

This paper examines the steps, systems, and processes necessary to overcome the challenges of inconsistent documentation.  Presented is one way to manage the volumes of information after the PHA team has completed their hazard analysis review.  The success of the review depends on each step that follows being carefully documented and managed giving the process “memory”.
