(76c) A Decade of Loss of Containment Lessons – Incident Investigations by the US Chemical Safety Board

The United States Chemical Safety and Hazard Investigation Board (CSB) has investigated about 70 chemical incidents since 1998. Most of the incidents involve loss of containment. Learning from those incidents can prevent similar incidents at other sites. This study reviews the many factors that led to loss of containment and the recommendations to prevent similar incidents. The study relates the factors and recommendations to safety management topics such as the PSM elements. Recognizing those relationships reveals more than a decade of lessons for preventing loss of containment. The study includes all CSB investigations and publications issued to date. The presentation also shows the historical impact of a few incidents to demonstrate the importance of maintaining containment.
